For the great majority, pregnancy and childbirth should be a positive and happy experience that culminates in a healthy mother and baby. This means, however, that on those occasions when things do go wrong, the effects are even more devastating than in other areas of healthcare. Maternity care must reconcile these dual aspects in order to be safe, effective and responsive. When it does not, the consequences may be stark. This Report details a distressing chain of events that began with serious failures of clinical care in the maternity unit at Furness General Hospital, part of what became the University Hospitals of Morecambe Bay NHS Foundation Trust. The result was avoidable harm to mothers and babies, including tragic and unnecessary deaths. What followed was a pattern of failure to recognise the nature and severity of the problem, with, in some cases, denial that any problem existed, and a series of missed opportunities to intervene that involved almost every level of the NHS. These events have finally been brought to light thanks to the efforts of some diligent and courageous families, who persistently refused to accept what they were being told. Those families deserve great credit. This Report includes detailed and damning criticisms of the maternity unit, the Trust and the regulatory and supervisory system. This Report sets out why that is and how it could have been avoided.

The Future of Nursing explores how nurses' roles, responsibilities, and education should change significantly to meet the increased demand for care that will be created by health care reform and to advance improvements in America's increasingly complex health system. At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. The purpose of this study was to investigate the transition experience of new graduate nurses during their first work experience after graduating from a baccalaureate undergraduate nursing program. A case study approach was used to examine a community hospital in Connecticut. A random sample of baccalaureate prepared new graduate nurses who graduated in May 2007, and were in the first year of working as registered nurses, was chosen. The research question examined how new graduate nurses perceived the supports provided within their transition experience. The study examined the supports through Benner's (1982) developmental theory of nursing. The four supports included: support during orientation, support from experienced nurses, supports to develop time management skills and support from their undergraduate nursing education. The four research questions addressed each of these supports. The study used three methods of data collection. Twelve new graduate nurses completed the quantitative survey, eight participated in the individual interview and four new graduate nurses participated in the focus group interview. The conclusions included the following: new graduate nurses were supported by the learning environment of the orientation; new graduate nurses perceived a supportive network was available to them within the hospital environment; time management skills are essential for the transition to entry nursing practice; new graduate nurses were assisted by their undergraduate school experiences during the transition period. Recommendations for practice have implications for making new graduates successful in a nursing career. Hospital orientations should provide preceptors who encourage autonomy and are consistent and provide emotional support and need to provide support in the transition period by mentoring and encouraging guidance from experienced nurses. Hospitals also need to provide clinician guidance and organizational resources to assist with time management, and offer intern programs to students prior to graduation. Undergraduate nursing programs should incorporate more clinical time to help the new graduate nurse transition into nursing practice. Recommendations for further research might involve a qualitative in-depth study regarding time management as well as an in depth case study examining new graduate nurses perceived level of support during the orientation period.

A nurse residency program (NRP) was developed to transition the graduate registered nurse (GRN) into clinical practice beyond the academic role. This quasi-experiment project examined new graduate nurses and how an NRP was implemented to assist them in transitioning from scholarly practice into the clinical practice role. Sixteen recent baccalaureate-prepared nurses participated in the program. Each participant agreed to enter the program as a part of their employment process with the facility. During this time, the graduate was supported by a seasoned mentor assisting them in developing critical thinking, improving communication skills, and applying clinical skills during real patient scenarios. The graduate met monthly with the program cohort for one hour to review evidence-based practices, clinical scenarios, and to participate in the didactic discussion. The topics included addressing the roles of the nurse: fatigue- countermeasures related to shift work, nursing documentation, skin assessment, lab interpretation, respiratory system and equipment review, evidence-based practices, and delegation. A paired sample t-test was used to compare pre- and post-test survey results using a questionnaire established by the project hospital. Data analysis identified NRP impacted participants’ responses to questions related to anticipated growth and development of skills, collaborative clinical care, application of knowledge in evidence-based practices, and expansion of knowledge and comfort levels when working in different areas of the facility.
