Excerpt from The Ophthalmoscope, Vol. 5: A Monthly Review of Current Ophthalmology; April 1, 1907 My reply to the second question is emphatically in the negative for any skilful surgeon. I have operated on about cataracts, and may therefore Claim to have met with most of the common complications that embarrass the ophthalmic surgeon. I can only say that, weighing, on the one hand, the dangers of couching, and, on the other, the so-called indications for its performance, I can think of no conditions which would lead me to undertake an operation which I regard both as unscientific and as opposed to the dictates of sound surgery. I am aware that this can Claim to be nothing more than an expression of opinion, but this opinion has not been lightly formed. For many years past, I have watched the couchers' results, keeping constantly before me the views of those who differ so widely from me on this question, and my conclusions have been arrived at, after very carefully weighing all sides of a question which I have felt to be of no small importance to those who practise eye-surgery in the East. In accepting the invitation of the Editor to undertake this task, I did so because, having seen Major Smith perform his operation at Jullundur nearly 200 times, I became convinced that, in several important particulars, it materially differed from that I had seen done by various surgeons and had performed myself, under the impression that Smith's teaching was being followed. It is certainly a wonderful sight to go on a pilgrimage to jullundur, and see the Civil Hospital, with 400 cataract patients, during those six weeks of autumn or spring when Smith has what he calls his throng-season. Excerpt from The Ophthalmoscope, Vol. 5: A Monthly Review of Current Ophthalmology; December 1, 1907 Matters are comparatively simple when the patient manifests bunches of enlarged glands in his neck or elsewhere, and the diagnostic value of this evidence is accentuated when anatomically unrelated groups of glands share in the change, for which no obvious and adequate cause can be made out. Dr. C. Stedman Bull4 has lately stated that enlarged glands are present in about 85 per cent. Of the cases Of anterior tuberculosis of the eye. A history of tubercle in the patient's family cannot be held to count for a great deal when it is remembered that such a history can be obtained in about one-half of all cases brought to a children's hospital for treatment. Apart from this practical consideration, the family inheritance Of tubercle does not nowadays rank so highly from a diagnostic point of view as it did a few years ago.
