Synopsis Scabs to Scars by : Paul Pierce

Many relationships are doomed from the start because we carry the baggage of previous hurts and disappointments into the new relationships. The hurts of the past must be dealt with so we can be free to love the way we really desire. Scabs are the product of wounds still in need of healing. When a hurt is truly healed, a scar forms. This scar reminds us of that hurt and the situation that caused the wound but the pain is gone. Life can bring us hurt but how we deal with those hurts determine if we are free and whole.

Synopsis Scar Management - ECAB by : Niti Khunger

A number of scar revision techniques can be utilized to treat specific types of scars, in combination with each other or with adjunctive therapies to achieve optimal results. Pathological conditions like hypertrophic scars and keloids are still challenging. Though a number of therapies exist to treat keloids, such as intralesional steroids and anti-metabolites, cryosurgery, to name a few, none are definitive in treatment. Understanding the molecular basis of keloids may lead to development of new therapies. Striae, also known as ‘stretch marks’, are extremely common, disfiguring and a real challenge to treat. Management of striae is a neglected field of research. Except for topical retinoids, the efficacy satisfactorily. Current treatment options include chemical peels, lasers and collagen induction therapy, with a tendency towards active intervention. However, studies are necessary to determine treatment strategies which produce results that are effective with minimal adverse effects. Post-acne scarring is one of the most common causes of facial scars, causing considerable cosmetic disability. Acne is a common skin disorder, unfortunately affecting a large number of teenagers. Usually patients have a combination of different types of scars; hence no single modality is useful. A combination of procedures is generally required for a satisfactory cosmetic outcome. The use of fractional lasers is gaining importance, as it is a minimally invasive procedure as compared to aggressive resurfacing procedures of the past. Collagen induction therapy is another promising technique. Today, patients are also getting up-to-date information about the various modalities available for scar treatment through media, such as magazines, advertisements and internet. However, exaggerated advertisements and information from nonprofessional sources, particularly with regard to lasers can influence patient expectations and result in unrealistic expectations. Patients need to understand that deep scars cannot be eliminated fully and can only be improved. They usually have high expectations from procedural techniques. Proper counseling is extremely important for a happy and satisfactory outcome. Current innovations and techniques in scar prevention and management and increasing use of minimally invasive surgeries are paving the way toward smoother, less noticeable scars. Every surgeon practicing aesthetics should be well versed with modalities of scar management and this is the aim of this clinical update.

It comprehensively addresses the concept of wound healing, from the healing process to debridement concepts, and various antiseptics and local antibiotics used in dressing materials to facilitate healing. The book also discusses the latest inventions and treatment options that have revolutionized wound healing, such as: oxygen therapy, ozone therapy, hyperbaric oxygen therapy, electric therapy and ultrasonic wound therapy. In closing, it examines the latest regenerative therapies based on stem cell therapy, cellular therapy and gene therapy. Given its scope, the book offers a valuable resource for medical students and physicians dealing with wound management, as well as a reference guide for nurses in primary and tertiary wound care settings.

Synopsis Burn Care for General Surgeons and General Practitioners by : David G. Greenhalgh

This textbook on burn care is focused specifically on the needs of a surgeon. It provides a “how to do” text that presents the practical strategies for initial resuscitation, skin grafting, burn-specific critical care and ultimately guide the surgeon for the best outcomes for the burn surgeon. It provides the reader with basic pathophysiology but avoids a detailed review of the molecular science of burns. Guidelines for basic care with the chapters covering the timeline used for each patient spanning from initial resuscitation (ABCs) to managing their re-socialization are provided. The text covers the key components to the initial management of the burn patient including airway, breathing and circulation. The reader is introduced to the metabolic changes of a major burn and how nutrition and pharmacologic manipulation affects these systemic effects of a burn injury. The management of burn sepsis as well as the basic concepts of wound healing of the different depths and severity of burns are covered. Another chapter covers the basic techniques of skin grafting, rehabilitation and reconstruction with a focus to optimize the cosmetic and functional outcome of the burn. Special chapters cover management of outpatient burns and pediatric injuries. Special burns – electrical, chemical, cold injuries and finally the treatment of other diseases that lead to massive skin loss are covered. Special considerations for pain, scar management, psychosocial recovery, intentional burns, burn prevention and disaster preparation complete the text. Burn Care for General Surgeons and General Practitioners provides a reference for general and plastic surgeons who are planning a career in burns. The textbook will also be a straightforward resource for the general or plastic surgeon who takes care of burns as part of their practice.
