Ebola is a rare and deadly disease. Since late 2013, West Africa has experienced the largest Ebola outbreak ever recorded. We pay tribute to all those who worked tirelessly to tackle this outbreak, some of whom gave evidence to this inquiry, and many of whom continue working to avert similar crises in the future. We also commend the Government on its leading contribution to the fight against Ebola, and the financial, and personnel, commitments that it made, from constructing and staffing Ebola treatment centres in Sierra Leone to deploying troops, helicopters, aircrew and an aviation support ship to provide much needed logistical support. Examples of UK successes in tackling Ebola, however, must not allow complacency to set in. Despite this impressive deployment of resources to combat Ebola in Sierra Leone, the UK response - like the international response - was undermined by systemic delay. The biggest lesson that must be learnt from this outbreak of Ebola is that even minor delays in responding cost lives. Yet delays were evident at every stage of our response, from escalating Public Health England's disease surveillance data to those with the capacity to act, to convening a Scientific Advisory Group for Emergencies which failed to be established until October 2014, three months after 'Cobra', the Government's emergency response committee, first met. In the absence of established mechanisms, ad hoc approaches emerged to fill the gaps. Inevitably, these were not as effective, or as targeted, as they should have been.

This book provides an engaging, jargon-free introduction to the threat of global pandemics, offering an overview of the many origins and triggers of pandemic events. It covers the impacts generated by novel infectious disease outbreaks across various dimensions – from social and ethical to medical and political, from media to economic and legal implications. The author discusses the preparedness strategies developed globally, the lessons learned from various outbreaks and the mitigation measures deployed — from quarantine and social distancing to data sharing and surveillance systems — including their unintended impacts. While the risk of global pandemics is certainly intensely debated by the scientific community, and increasingly by policy makers at various levels, the threat is hardly discussed in the public domain. It only permeates the media during crisis events, such as during the SARS outbreak in 2003, the West African Ebola outbreak in 2014–15, and most notably the ongoing COVID-19 global pandemic crisis. This book is thus highly timely and topical. It has a global scope, whilst at times zooming in on the implications of pandemic risk and mitigation for the Global North or the Global South. Given the interdisciplinarity of the topic, this book will be of great interest to a wider non-academic audience, as well as students from a range of subjects including politics, sociology, geography, anthropology, and international development, along with entry-level medical students keen to widen their appreciation of the social dimensions of the medical work they set out to conduct.

The 2014â€"2015 Ebola epidemic in western Africa was the longest and most deadly Ebola epidemic in history, resulting in 28,616 cases and 11,310 deaths in Guinea, Liberia, and Sierra Leone. The Ebola virus has been known since 1976, when two separate outbreaks were identified in the Democratic Republic of Congo (then Zaire) and South Sudan (then Sudan). However, because all Ebola outbreaks prior to that in West Africa in 2014â€"2015 were relatively isolated and of short duration, little was known about how to best manage patients to improve survival, and there were no approved therapeutics or vaccines. When the World Heath Organization declared the 2014-2015 epidemic a public health emergency of international concern in August 2014, several teams began conducting formal clinical trials in the Ebola affected countries during the outbreak. Integrating Clinical Research into Epidemic Response: The Ebola Experience assesses the value of the clinical trials held during the 2014â€"2015 epidemic and makes recommendations about how the conduct of trials could be improved in the context of a future international emerging or re-emerging infectious disease events.

The draft Investigatory Powers Bill was published by the Government on 4 November 2015. Ministers have been clear that the intention of this Bill is to consolidate and clarify existing legislation on the interception of communications and the acquisition of communications data and to modernise the law in the light of developments in communications technologies, in order to maintain the operational capabilities of law enforcement agencies and the intelligence and security services. Previous attempts to legislate in this area have met with criticisms over the lack of consultation with communications service providers (CSPs) on matters of technical feasibility and cost. In our inquiry we have focused on technological aspects of the draft Bill in order to identify the main technological issues involved and how these might affect the communications businesses that will have to collect data and cooperate with the security authorities. If law enforcement agencies and the intelligence and security services are effectively to combat terrorism and serious crime, they must have the means to keep pace with developments in communications. They will doubtless need to continue to deploy a range of methods for intercepting and acquiring information about communications. The evidence we have received suggests there are still many unanswered questions about how this legislation will work in the fast moving world of technological innovation. It is essential that the integrity and security of legitimate online transactions is maintained if we are to trust in, and benefit from, the opportunities of an increasingly digital economy.

In the past half century, deadly disease outbreaks caused by novel viruses of animal origin - Nipah virus in Malaysia, Hendra virus in Australia, Hantavirus in the United States, Ebola virus in Africa, along with HIV (human immunodeficiency virus), several influenza subtypes, and the SARS (sudden acute respiratory syndrome) and MERS (Middle East respiratory syndrome) coronaviruses - have underscored the urgency of understanding factors influencing viral disease emergence and spread. Emerging Viral Diseases is the summary of a public workshop hosted in March 2014 to examine factors driving the appearance, establishment, and spread of emerging, re-emerging and novel viral diseases; the global health and economic impacts of recently emerging and novel viral diseases in humans; and the scientific and policy approaches to improving domestic and international capacity to detect and respond to global outbreaks of infectious disease. This report provides a synthetic view of national risk assessments (NRAs) in twenty OECD Member countries. NRA are used to support risk management decisions in a rapidly changing global risk landscape characterized by increasingly complex, interconnected societies and highly mobile people, information and goods. The report highlights good governance practices in establishing NRAs and how the results are used to inform public policy. It identifies challenges that OECD Member countries continue to confront in their efforts to implement NRA, and makes concrete recommendations where improvements could still be made.

Molecular epidemiology has recently broaden its focuses due to the development of molecular tools but also by incorporating advances of other fields such as mathematical epidemiology, molecular ecology, population genetics and evolution. Facing new risks of emerging and re-emerging infectious diseases that are threats for humans and their livestock, the objectives of molecular epidemiology include: - the development of molecular tools, genotyping and gene expression - the incorporation of concepts and results of population genetics of infectious diseases - the integration of recent advances in theoretical epidemiology and evolutionary ecology of diseases - a better understanding of transmission for the development of risk factors analyses. This book will demonstrate how the latest developments in molecular tools and in epidemiology can be integrated with studies of host-pathogen interactions. Besides a strong theoretical component, there will also be an emphasis on applications in the fields of epidemiology, public health, veterinary medicine, and health ecology. Recent disease events such as SARS, H1N1 and avian influenza, and haemorrhagic fevers have focussed policy and public concern as never before on epidemics and so-called 'emerging infectious diseases'. Understanding and responding to these often unpredictable events have become major challenges for local, national and international bodies. All too often, responses can become restricted by implicit assumptions about who or what is to blame that may not capture the dynamics and uncertainties at play in the multi-scale interactions of people, animals and microbes. As a result, policies intended to forestall epidemics may fail, and may even further threaten health, livelihoods and human rights. The book takes a unique approach by focusing on how different policy-makers, scientists, and local populations construct alternative narratives-accounts of the causes and appropriate responses to outbreaks- about epidemics at the global, national and local level. The contrast between emergency-oriented, top-down responses to what are perceived as potentially global outbreaks and longer-term approaches to diseases, such as AIDS, which may now be considered endemic, is highlighted. Case studies-on avian influenza, SARS, obesity, H1N1 influenza, HIV/AIDS, tuberculosis, and haemorrhagic fevers-cover a broad historical, geographical and biological range. As this book explores, it is often the most vulnerable members of a population-the poor, the social excluded and the already ill-who are likely to suffer most from epidemic diseases. At the same time, they may be less likely to benefit from responses that may be designed from a global perspective that neglects social, ecological and political conditions on the ground. This book aims to bring the focus back to these marginal populations to reveal the often unintended consequences of current policy responses to epidemics. Important implications emerge - for how epidemics are thought about and represented; for how surveillance and response is designed; and for whose knowledge and perspectives should be included. SARS, Governance and the Globalization of Disease provides a comprehensive and original analysis of the historic global SARS outbreak of 2003. David P. Fidler constructs a political pathology of the SARS outbreak, analyzes the government responses to it, places these responses in historical context and assesses the implications of the successful management of the outbreak for handling future pathogenic threats that will arise. The book includes a detailed description of the outbreak and governance responses to it, as well as a focused analysis of China's role in the outbreak.
