The Handbook focuses on an overview of key issues which should be of concern to prison managers and the reforms they must often engage in and promote as prison leaders. It is meant to support a basic five-day training workshop for prison officials responsible for leading and managing prisons in developing and post-conflict countries. It is aimed to explore and understand practical ways in which prison leaders can more effectively implement international standards and norms in the institutions for which they are responsible. The Handbook and the workshop curriculum provide a template to help leaders identify the changes required in their environment and to reflect on the challenges they are likely to encounter in bringing about these changes.

Current estimates indicate that approximately 2.2 million people are incarcerated in federal, state, and local correctional facilities across the United States. There are another 5 million under community correctional supervision. Many of these individuals fall into the classification of special needs or special populations (e.g., women, juveniles, substance abusers, mentally ill, aging, chronically or terminally ill offenders). Medical care and treatment costs represent the largest portion of correctional budgets, and estimates suggest that these costs will continue to rise. In the community, probation and parole officers are responsible for helping special needs offenders find appropriate treatment resources. The Primeiro Comando do Capital (PCC) is a Sao Paulo prison gang that since the 1990s has expanded into the most powerful criminal network in Brazil. Karina Biondi's rich ethnography of the PCC is uniquely informed by her insider-outsider status. Prior to his acquittal, Biondi's husband was incarcerated in a PCC-dominated prison for several years. During the period of Biondi's intense and intimate visits with her husband and her extensive fieldwork in prisons and on the streets of Sao Paulo, the PCC effectively controlled more than 90 percent of Sao Paulo's 147 prison facilities. Available for the first time in English, Biondi's riveting portrait of the PCC illuminates how the organization operates inside and outside of prison, creatively elaborating on a decentered, non-hierarchical, and far-reaching command system. This system challenges both the police forces against which the PCC has declared war and the methods and analytic concepts traditionally employed by social scientists concerned with crime, incarceration, and policing. Biondi posits that the PCC embodies a "politics of transcendence," a group identity that is braided together with, but also autonomous from, its decentralized parts. This handbook aims to assist legislators, policymakers, prison managers, staff and non-governmental organizations in implementing international standards and norms related to the gender-specific needs of women prisoners, in particular the United Nations Rules for the Treatment of Women Offenders and Non-Custodial Measures for Women Offenders ('the Bangkok Rules'). It further aims to increase awareness about the profile of female offenders and to suggest ways in which to reduce their unnecessary imprisonment, including by rationalizing legislation and criminal justice policies, and by providing a wide range of alternatives to prison at all stages of the criminal justice process.
