This issue of Clinics in Laboratory Medicine, edited by Drs. Nahed Ismail, A. William Pasculle, and James Snyder, will cover a wide variety of Emerging Pathogens. Topics covered in this issue include, but are not limited to West Nile Virus; Zika Virus; Ebola and Marburg Heamorrhagic Fever; Rift Valley Fever; Carbapenem Resistant Enterobacteriacae; Closteridium Difficuile; and Chikungunya, among others.

The Forum on Emerging Infections was created in 1996 in response to a request from the Centers for Disease Control and Prevention and the National Institutes of Health. The goal of the forum is to provide structured opportunities for representatives from academia, industry, professional and interest groups, and government to examine and discuss scientific and policy issues that relate to research, prevention, detection, and management of emerging infectious diseases. A critical part of this mission has been the convening of a series of workshops. Public Health Systems and Emerging Infections summarizes the fourth in a series of five workshops. With a focus on our knowledge and understanding of the role of private and public health sectors in emerging infectious disease surveillance and response, the participants explored the effects of privatization of public health laboratories and the modernization of public health care. The issues discussed included epidemiological investigation, surveillance, communication, coordination, resource allocations, and economic support.

This issue on Diagnostic Testing for Enteric Patholgens is led by two experts in the field of clinical pathology: Alexander J. McAdam and Collette Fitzgerald. Topics include Salmonella, Shigella and Yersinia; Escherichia coli; Campylobacter; Clostridium difficile; Use of markers of intestinal inflammation for diagnosis of infectious gastroenteritis; Antibiotic susceptibility testing of bacteria that cause gastroenteritis; Norovirus; Rotavirus; Intestinal Abebae; Intestinal coccidia and cryptosporidium; Intestinal microsporidia; and Multiplex PCR tests for gastroentieritis. An added features of this issues a Q and A on a controversial area in clinical microbiology, related to STI testing. Several participants from different fields each answer the same series of questions; specialists from clinical laboratory medicine, public health and clinical patient care participate. Each question is introduced by the lead Editors.

In October 1999, the Forum on Emerging Infections of the Institute of Medicine convened a two-day workshop titled "International Aspects of Emerging Infections." Key representatives from the international community explored the forces that drive emerging infectious diseases to prominence. Representatives from the Americas, Africa, Asia and the Pacific, and Europe made formal presentations and engaged in panel discussions. Emerging Infectious Diseases from the Global to the Local Perspective includes summaries of the formal presentations and suggests an agenda for future action. The topics addressed cover a wide range of issues, including trends in the incidence of infectious diseases around the world, descriptions of the wide variety of factors that contribute to the emergence and reemergence of these diseases, efforts to coordinate surveillance activities and responses within and across borders, and the resource, research, and international needs that remain to be addressed.

Infectious diseases are a global hazard that puts every nation and every person at risk. The recent SARS outbreak is a prime example. Knowing neither geographic nor political borders, often arriving silently and lethally, microbial pathogens constitute a grave threat to the health of humans. Indeed, a majority of countries recently identified the spread of infectious disease as the greatest global problem they confront. Throughout history, humans have struggled to control both the causes and consequences of infectious diseases and we will continue to do so into the foreseeable future. Following up on a high-profile 1992 report from the Institute of Medicine, Microbial Threats to Health examines the current state of knowledge and policy pertaining to emerging and re-emerging infectious diseases from around the globe. It examines the spectrum of microbial threats, factors in disease emergence, and the ultimate capacity of the United States to meet the challenges posed by microbial threats to human health. From the impact of war or technology on disease emergence to the development of enhanced disease surveillance and vaccine strategies, Microbial Threats to Health contains valuable information for researchers, students, health care providers, policymakers, public health officials. and the interested public.

The field of Clinical Microbiology is evolving at a rapid pace, perhaps more so than any other arm of laboratory medicine. This can be attributed to new technology, including high throughput gene sequencing, multiplex molecular assays, rapid evolution of antimicrobial resistance, and discovery of new pathogens. In addition, modern medical procedures, such as solid organ and stem cell transplantation, have resulted in an explosion of infections with agents that historically have been considered to be of low virulence. This issue of Clinics in Laboratory Medicine will highlight some of the advances in diagnostic microbiology, including MALDI-TOF MS, pathogen discovery, and personalized antimicrobial chemotherapy. In addition, one of the papers will focus on implementation of new technologies and how to maximize patient impact of these new methods.

The Human Biome is a complex and essential biological system within the human body. The adult human harbors some 100 trillion bacteria in his gut alone. Balance of this system is essential to good health. This issue of Clinics in Laboratory Medicine focuses on treatments to realign this balance as well as advances in understanding the system in general.
