Our series Cancer Prevention - Cancer Control continues to address the causes and prevention of cancer. In this volume, Hill, Elwood, and English bring together a rich resource summarizing the state of science underpinning the primary prevention of skin cancer. While skin cancer causes an increasing burden, particularly in populations of European origin, our understanding of the role of sun exposure together with the genetic components of skin cancer continues to grow. Given the emphasis on evidence-based medicine and public health prevention efforts, it is noteworthy that, although we can all access the same evidence base, countries around the world have had remarkably different responses to the application of this knowledge to prevent skin cancer. The outstanding contribution of the Australian public health community to the scientific understanding of skin cancer etiology and the translation of this knowledge into national prevention efforts uniquely positions the editors to compile this volume focused on the primary prevention of skin cancer. In so doing they draw on an international team of authors to present a “state of the science” summary of skin cancer prevention and to identify those areas where uncertainty remains. This document is a Call to Action to partners in prevention from various sectors across the nation to address skin cancer as a major public health problem. Many partners are essential to this effort, including federal, state, tribal, local, and territorial governments; members of the business, health care, and education sectors; community, nonprofit, and faith-based organizations; and individuals and families. The goal of this document is to increase awareness of skin cancer and to call for actions to reduce its risk.The first section describes the problem of skin cancer and its major risk factors. It also discusses the relationship between exposure to ultraviolet (UV) radiation and health. The second section describes the current evidence on preventing skin cancer, including current initiatives in the United States and in other countries. The third section describes the gaps in research related to skin cancer prevention, highlighting areas of research where more work is needed. The fourth section identifies specific opportunities to prevent skin cancer by reducing UV exposure in the U.S. population and calls for nationwide action.

"Sun Protection in Man" looks at the beneficial and harmful effects of solar radiation. The physiological consequences of sun exposure have been systematically studied starting at the end of the nineteenth century and we now have accumulated knowledge about how Caucasian and Asian skins reacts to solar radiation. The chemical effects of solar ultraviolet radiation have been analyzed with particular emphasis during the second half of the twentieth century. Research on micro-organisms has allowed us to understand the mechanisms of UV-induced mutagenesis and photosensitization. Studies with laboratory rodents have opened the path to the understanding of UV-induced immune-depression, carcinogenesis, photo-damage and photo-aging. The results of these studies have enabled other scientists to investigate the same phenomena in human organs such as the skin and the eye. UV radiation damages hair, as well. The present knowledge in these fields is summarized in some of the chapters of this monograph.Mass phenomenon in Europe with the generalization of summer vacations which were a consequence of social reforms introducing the concept of "paid vacations". This created a need for protection and opened a market for sunscreens. This monograph is concerned with sun protection as a whole and is not just "another book on sunscreens". Nonetheless, in these days of general concern, it is important to learn about the efficiency of sunscreens. Several authors discuss how to reduce the number of impinging photons and explain why sunscreens seem to offer less protection than expected. Guidelines are given on how to use sunscreens in everyday life, which are expressed rigorously though clearly, for access to the common reader.Our knowledge on the relationship between sun and humans is at the early stages of development. Industrial and commercial activities are concerned by the development of this knowledge, and rules have been and will be promulgated to guarantee efficacy and safety of sun-products. The concept of expressing acidity as the negative logarithm of the hydrogen ion concentration was defined and termed pH in the beginning of the 20th century. The general usefulness of the pH concept for life science was recognized and later gained importance to analytical research. Reports on results of pH measurements from living skin established the term acid mantle - the skin's own protective shield that maintains a naturally acid pH. It is invisible to the eye but crucial to the overall wellbeing of skin. Chronic alkalization can throw this acid mantle out of balance, leading to inflammation, dermatitis, and atopic skin diseases. It is therefore no surprise, that skin pH shifts have been observed in various skin pathologies. It is also obvious that the pH in topically applied preparations may play an important role. Optimal pH and buffer capacity within topical preparations not only support stability of active ingredients and auxiliary materials, but may also increase absorption of the non-ionized species of an acidic or a basic active ingredient. They may even open up opportunities to modify and "correct" skin pH and hence accelerate barrier recovery and maintain or enhance barrier integrity. Further efforts are needed to standardize and improve pH measurements in biological media or pharmaceutical/cosmetic vehicles to increase and ensure quality, comparability, and relevance of research data.
