Advances in trauma care have accelerated over the past decade, spurred by the significant burden of injury from the wars in Afghanistan and Iraq. Between 2005 and 2013, the case fatality rate for United States service members injured in Afghanistan decreased by nearly 50 percent, despite an increase in the severity of injury among U.S. troops during the same period of time. But as the war in Afghanistan ends, knowledge and advances in trauma care developed by the Department of Defense (DoD) over the past decade from experiences in Afghanistan and Iraq may be lost. This would have implications for the quality of trauma care both within the DoD and in the civilian setting, where adoption of military advances in trauma care has become increasingly common and necessary to improve the response to multiple civilian casualty events. Intentional steps to codify and harvest the lessons learned within the military's trauma system are needed to ensure a ready military medical force for future combat and to prevent death from survivable injuries in both military and civilian systems. This will require partnership across military and civilian sectors and a sustained commitment from trauma system leaders at all levels to assure that the necessary knowledge and tools are not lost. A National Trauma Care System defines the components of a learning health system necessary to enable continued improvement in trauma care in both the civilian and the military sectors. This report provides recommendations to ensure that lessons learned over the past decade from the military's experiences in Afghanistan and Iraq are sustained and built upon for future combat operations and translated into the U.S. civilian system.

As seen in military documents, medical journals, novels, films, television shows, and memoirs, soldiers’ invisible wounds are not innate cracks in individual psyches that break under the stress of war. Instead, the generation of weary warriors is caught up in wider social and political networks and institutions—families, activist groups, government bureaucracies, welfare state programs—mediated through a military hierarchy, psychiatry rooted in mind-body sciences, and various cultural constructs of masculinity. This book offers a history of military psychiatry from the American Civil War to the latest Afghanistan conflict. Preventing humanitarian atrocities is becoming as important for the United Nations as dealing with inter-state war. In this book, Ramesh Thakur examines the transformation in UN operations, analysing its changing role and structure. He asks why, when and how force may be used and argues that the growing gulf between legality and legitimacy is evidence of an eroded sense of international community. He considers the tension between the US, with its capacity to use force and project power, and the UN, as the centre of the international law enforcement system. He asserts the central importance of the rule of law and of a rules-based order focused on the UN as the foundation of a civilised system of international relations. In 2015, Congress tasked the Department of Defense to commission an independent assessment of U.S. military strategy and force posture in the Asia-Pacific, as well as that of U.S. allies and partners, over the next decade. This CSIS study fulfills that congressional requirement. The authors assess U.S. progress to date and recommend initiatives necessary to protect U.S. interests in the Pacific Command area of responsibility through 2025. Four lines of effort are highlighted: (1) Washington needs to continue aligning Asia strategy within the U.S. government and with allies and partners; (2) U.S. leaders should accelerate efforts to strengthen ally and partner capability, capacity, resilience, and interoperability; (3) the United States should sustain and expand U.S. military presence in the Asia-Pacific region; and (4) the United States should accelerate development of innovative capabilities and concepts for U.S. forces.

Dedicated to the Sailors and Marines who lost their lives on the final voyage of USS Indianapolis and to those who survived the torment at sea following its sinking. plus the crews that risked their lives in rescue ships. The USS Indianapolis (CA-35) was a decorated World War II warship that is primarily remembered for her worst 15 minutes. . This ship earned ten (10) battle stars for her service in World War II and was credited for shooting down nine (9) enemy planes. However, this fame was overshadowed by the first 15 minutes July 30, 1945, when she was struck by two (2) torpedoes from Japanese submarine I-58 and sent to the bottom of the Philippine Sea. The sinking of Indianapolis and the loss of 880 crew out of 1,196 --most deaths occurring in the 4-5 day wait for a rescue delayed --is a tragedy in U.S. naval history. This historical reference showcases primary source documents to tell the story of Indianapolis, the history of this tragedy from the U.S. Navy perspective. It recounts the sinking, rescue efforts, follow-up investigations, aftermath and continuing communications efforts. This document presents key messages and the state-of-the-art of soil pollution, its implications on food safety and human health. It aims to set the basis for further discussion during the forthcoming Global Symposium on Soil Pollution (GSOP18), to be held at FAO HQ from May 2nd to 4th 2018. The publication has been reviewed by the Intergovernmental Technical Panel on Soil (ITPS) and contributing authors. It addresses scientific evidences on soil pollution and highlights the need to assess the extent of soil pollution globally in order to achieve food safety and sustainable development. This is linked to FAO’s strategic objectives, especially SO1, SO2, SO4 and SO5 because of the crucial role of soils to ensure effective nutrient cycling to produce nutritious and safe food, reduce atmospheric CO2 and N2O concentrations and thus mitigate climate change, develop sustainable soil management practices that enhance agricultural resilience to extreme climate events by reducing soil degradation processes.
