During the past decades, with the introduction of the recombinant DNA, hybridoma and transgenic technologies there has been an exponential evolution in understanding the pathogenesis, diagnosis and treatment of a large number of human diseases. The technologies are evident with the development of cytokines and monoclonal antibodies as therapeutic agents and the techniques used in gene therapy. Immunopharmacology is that area of biomedical sciences where immunology, pharmacology and pathology overlap. It concerns the pharmacological approach to the immune response in physiological as well as pathological events. Local treatment cures about 30 to 40% of cancers, this proportion depending on the follow-up required to establish it. This means that 60 to 70% of the malignant neoplasias are disseminated either perceptibly (leukemias, visible metas tases) or imperceptibly, forming a 'minimal imperceptible disease', which local treatment leaves, whether it consists of surgery, radiotherapy, or surgery plus radiotherapy. When the neoplastic tissue is voluminous enough to be per ceptible, cures can be obtained with chemotherapy or chemo immunotherapy. When the neoplastic disease is imperceptible, made up of micrometastases, it apparently can be cured by systemic postsurgical chemotherapy, immunotherapy, or chemoimmunotherapy. Hence there is the need for intensive development of these medical therapies which are applied by the medical oncol ogist and, at present, consist of chemotherapy, immuno therapy, or chemoimmunotherapy. These medical thera peutics can only grow with scientific development, the main weapon of which is experimental and clinical pharmacology. The Analog Potential in Cancer Chemotherapy in the United States and Japan S. K. Carter Northern California Cancer Program, 1801 Page Mill Road, Building B, Suite 200, USA - Palo Alto, CA 94304 The bilateral collaborative program in cancer research between Japan and the United States is one of the oldest programs of this type. It is sponsored in the United States by the National Cancer Institute [1], while in Japan the sponsoring organization is the Japan Society for the Promotion of Science. Annual symposia concerning treatment have been held and in recent years they have been published [2, 3]. Drug development in both Japan and the United States has evolved over the years to an increasing emphasis on second generation compounds. This has come about as a result of the initial successes of chemotherapy development. Many active structures have been uncovered and the armamentarium of the medical and pediatric oncologist has grown dramatically. The uncovering of an active structure provides an opportunity for analog synthesis and attempts at elucidating structure-activity relationships. It is hoped that the therapeutic index of active structures can be improved so as to achieve superior clinical results.

On May 14-16, 1982, a group of scientists met in Viareggio, Italy to present ideas, and exchange relevant data on current approaches on immunomodulators. We felt that the embodiment of the substance of that international symposium into this volume will be beneficial in keeping both clinicians and basic scientists abreast of the latest exciting developments in this rapidly accelerating field. We hope that the publication of this series of papers by an international panel of experts will enhance the nature of future inves t igat ive stud ies with such ent it ies . Under the rubric of immunology, it is obvious that such a term as "immunomodulation" could be construed as being all encompassing. However, it is intriguing that the immune modulators touched on in these sessions can be subcategorized into about five broad groups. Almost all substances, excepting those entities that are immuno logically inert, will have some modulating effects on immunity. From the coarse adjustment of antibody feedback inhibition upon cellular limnunity to the subtle finesse envisioned in Jerne's idiotypic network and s'lppressor T cells, immunomodulators attempt to either magnify or diminish those responses that are normally elicited by antigen. The broad groups - thymic hormones, D.L.E., Interferon, drugs, and the use of bacterial products appear to be at the forefront of much of the pioneering work on immunomodulation.

The interplay between tumors and their immunologic microenvironment is complex, difficult to decipher, but its understanding is of seminal importance for the development of novel prognostic markers and therapeutic strategies. The present review discusses tumor-immune interactions in several human cancers that illustrate various aspects of this complexity and proposes an integrated scheme of the impact of local immune reactions on clinical outcome. Current active immunotherapy trials have shown durable tumor regressions in a fraction of patients. However, clinical efficacy of current vaccines is limited, possibly because tumors skew the immune system by means of myeloid-derived suppressor cells, inflammatory type 2 T cells and regulatory T cells (Tregs), all of which prevent the generation of effector cells. To improve the clinical efficacy of cancer vaccines in patients with metastatic disease, we need to design novel and improved strategies that can boost adaptive immunity to cancer, help overcome Tregs and allow the breakdown of the immunosuppressive tumor microenvironment.
